Apply to the village office (the service window is available as well) and transfer to the township office to issue.

Instruction for Name of Forms or Documents

1. Two application forms.

2. One affidavit.

3. One land registration transcript.

4. One zoning use certificate of urban planning. (bring your land ownership certificate and apply to Urban Planning Section, Economic Affairs Bureau )

5. Two sets of building photos (photos of the front, back, left and right each in every set) and and the house number must be taken or need to attach with address plate certification if no house number. 

6. Four building location maps (should be in cadastral map transcription)

7. Four sketch maps of building floor plan (length X width)
